*Name of sponsor(s): ( usually 1, but can be 2 - if Godparents or 2 people are favoured. Godparents are preferred, but a single sponsor is also acceptable and sponsors must be a practicing Catholics. Parents CANNOT be sponsors. Siblings can be sponsors, but they must be 18 or older. If a sponsor has not been selected at this time, then please advise David LeClair @ 892-1558 or daleclair@pei.eastlink.ca . A decision would need to be made no later than October 27th.
Student Workbook Each candidate will require a Student workbook ( Decision Point) and other supplies. There will be a fee involved in the preparation for Confirmation. The fee will be announced at a meeting of parents and candidates in mid-September. To register your child, please return this completed form to the parish office, OR register online at: www.holyredeemerpei.com no later than Sept 15th.

[Please note that there is a fee per family. Details will be announced before the frst class. It is usually paid through the teacher of the eldest child in the program.]
PLEASE NOTE If a candidate was not baptized at Holy Redeemer, then a baptismal certificate must be supplied by November 29th along with the name and address of the church of baptism. Proof of Baptism must be supplied before one can receive any sacrament other than baptism. The name and address of the church of baptism is needed because church regulations require that all statistics on a Catholic be forwarded to and recorded in the church in which he/she was baptized.
